I have the Form module by Nova Code. How do i get a button on the website for user to download forms data after they have submitted?

e.g say we a are giving out scholarships to needy kids and we need a way they can download a pdf template preloaded with the data they just posted 2 seconds ago, aaaaand extra fields like 1) header with two cloumns one with our logo and info  other column with a QR image and 2)footer with two columns one for a Local authority figure from their area to make a signature other column for stamp.
